Specialist Support

Coordination

A highest level of support coordination, reserved for participants with highly complex needs or challenging situations. It is typically delivered by appropriately qualified professionals (e.g., social workers, occupational therapists, psychologists) with expertise in managing complex disability supports.
Our Specialist Support Coordinator will assist to:
  • Address complex barriers in the participant's support environment.

  • Manage high-risk situations and provide intensive, timely intervention.

  • Design complex service plans that integrate supports across multiple systems (NDIS, health, justice, education, etc.).

  • Coordinate a comprehensive and cohesive support team.

  • Provide expert guidance and advocacy in challenging circumstances.

  • Focus on crisis resolution, prevention, and mitigation.

  • To reduce complexity, overcome significant barriers, and ensure consistent delivery of services for participants with intricate and high-risk support needs.

Support Coordination
Our level 2 Support Coordinator will work closely with participants to:
  • Understand their NDIS plan and funding.

  • Identify and connect with a mix of formal, informal, and community-based supports & services.

  • Design and establish support approaches.

  • Coach and refine the participant's skills in managing their supports.

  • Monitor plan budgets and support effectiveness.

  • Help resolve issues and manage relationships with service providers.

  • Assist in preparing for plan reviews.

  • Provide crisis planning, prevention, mitigation, and action.

Goal:

  • To strengthen a participant's ability to direct their own life, build a resilient network of supports, and take control of their NDIS plan.

Ideal For: Participants who are new to the NDIS
